Thank you for visiting First Capital's website. After many years providing and supporting various Commercial insurance products, we decided to shutdown the last of our programs, Transportation. As such, we are no longer writing new business, or renewing existing business.

CLAIMS

To report a claim, please contact claims@aegisinsco.com. Include the Named Insured and policy number in the email.

LOSS RUN REQUESTS

To request loss runs, send a signed authorization form to truckinglossruns@fcawis.com.

NOTE: Due to the sensitive nature of information on the reports you are requesting, First Capital does not accept digital stamp or electronic signatures of any type on Release Authorization Forms unless the digital signature audit trail or copy of the named insured’s driver’s license is provided.

We legally have 10 business days to respond to loss run requests. Do not send 2nd requests before the 10 days have passed. Sending multiple requests overloads the inbox and delays response times on all requests.

PAYMENTS

Online Payment

Submitting payment online is the quickest and most reliable way to ensure that premiums are received by our Accounting Department. You will receive a receipt of confirmation once you complete the transaction. Click here to submit payment online.

Mailing Payment

To mail a check payment to our office through USPS, address the envelope to our lockbox:
